United Concordia has negotiated our contract terms and conditions which enabled us to keep certain networks at our offices.  These plans are solely based on the contract between your employer/broker and United Concordia, which align more closely to our standard of care at this office.

IF YOU HAVE UNITED CONCORDIA/HIGHMARK INSURANCE, PLEASE CONTACT YOUR INSURANCE COMPANY TO INQUIRE IF YOU HAVE ONE OF THE ABOVE PLANS.  IF YOU NEED ASSISTANCE, PLEASE CONTACT OUR OFFICE.

If your insurance is not one of the above companies, you are more than welcome to stay as a loyal patient at our office while still taking advantage and using your insurance benefits.  As a courtesy to you, we will submit all of your claims and provide the required information and documentation to your insurance company for successful reimbursement.  Payment is due in full at the time services are rendered.  You can anticipate receiving a check in 2-4 weeks from your insurance company ranging from 50%-80% of your original payment depending on the services you received.  Please note that we cannot submit claims for State or Medicaid plans.

We have dedicated ourselves to providing personalized, high-quality care without the limitations of insurance.  This allows us to spend more time with each patient, use the best materials, and deliver care that's focused entirely on your comfort and needs.  Many of our patients appreciate the ease, flexibility, and elevated experience.  We will gladly prepare and submit your insurance claim for you so you can be reimbursed directly.

Any previous quotes for treatment will be voided on 1/1/2026.  HOWEVER, we are excited to implement updated state of the art technology in order to treat you more efficiently and comfortably.  Please inquire about our updated pricing for our Luminous services.

Should you choose to opt out of your employer’s insurance plan, please remember we offer our Smile Savers Club.  Plans START at $35 per month (plus $99 Lifetime Activation Fee).  This includes:

• TWO cleaning and check-up exams with complimentary Fluoride Treatments PER YEAR

• ONE emergency visit with complimentary x-rays PER YEAR

• 20% discount on ALL TREATMENT

• 25% discount on Invisalign
